Phoenix Athletic Performance/St. George's School Release

For use of athletic facilities (including, but not limited to the turf fields, Wellness Center, field house, gym, cold plunge) and equipment at St. George’s School with Phoenix Athletic Performance.   

The parties to this release have chosen to use facilities and equipment (including but not limited to Wellness Center, turf fields, field house, gym, cold plunge, etc) at St. George’s School and Phoenix Athletic Performance during 2026. 

Assumption of Risk    

I understand and accept that participation in the use of the chosen athletic facility may expose participants to increased physical risks. Some of the risks which may be present or occur include, but are not limited to, those associated with the following activities: the physical exertion associated with an exercise program; the risk of injury associated with the use of athletic facilities; the risk of injury associated with the use of athletic equipment or being near others using such equipment.  

Adherence to Standards  

Participant understands and agrees to abide by the following rules and regulations of the St. George’s School athletic facilities and Phoenix Athletic Performance. Participant further agrees to obey all rules, directions, and precautions issued by St. George’s School or its representatives; and Phoenix Athletic Performance by any associated individuals, institutions, or organizations; or by the State of Rhode Island or the Federal government.  

Release of Liability  

In consideration of use of athletic facilities (including, but not limited to, the turf fields, Wellness Center, field house, gym, cold plunge) and equipment, I have and do hereby release and hold harmless St. George’s School, Phoenix Athletic Performance and all its employees, trustees, faculty, representatives, and agents (hereafter, “released parties”) from any and all liability, actions, causes of action, claims, or demands of every kind and nature whatsoever and specifically any claim for negligence or negligent acts which may arise out of, or in connection with, the participant’s use of the athletic facilities (including, but not limited to, the turf fields, Wellness Center, field house and gym) and equipment. Participant further releases and discharges the released parties from responsibility for any accident, illness, injury, or any other consequences arising or resulting directly or indirectly from participant’s use of the athletic facilities and equipment. The participant recognizes and agrees that the released parties assume no responsibility for any liability, damage, or injury that may be caused by participant’s negligence or willful acts committed prior to, during, or after use of athletic facilities and equipment or for any liability, damage, or injury caused by the intentional or negligent acts or omissions caused by any other person. The terms hereof shall serve as a release, indemnification, and assumption of risk for the participant’s heirs, executors, representatives, attorneys, and administrators, and for all members of the participant’s family, including any minors. I agree that the foregoing obligation shall be binding. 

Governing Law 

This release shall be construed in accordance with, and enforced by, the laws of the State of Rhode Island, notwithstanding its choice of law principles.  

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between me and St. George’s School and sign it of my own free will.
